Information We Collect: We collect information you provide directly, including your name, contact information, and any information you share through our contact forms or client portal. Our website may also collect anonymized analytics data.

How We Use It: Information is used solely to provide services, respond to inquiries, and communicate with current and prospective clients. We do not sell, rent, or share your personal information with third parties except as required by law.

Contact Forms & Email: Please do not submit confidential, urgent, or clinical information through our website contact forms or general email. These channels are not encrypted or monitored for clinical emergencies.

Cookies & Analytics: Our website may use cookies and analytics tools to improve user experience. No personally identifiable information is collected through these tools.

HIPAA

Notice of Privacy Practices

As a HIPAA-covered entity, Breezer Counseling & Wellness is required to maintain the privacy of your protected health information (PHI) and to provide you with this notice of our legal duties and privacy practices.

How We Use and Disclose PHI: We use your health information to provide treatment, obtain payment, and conduct healthcare operations. We will not disclose your information without your written authorization except where required or permitted by law, including mandatory reporting obligations under Illinois law.

Mandated Reporting: As licensed clinicians in Illinois, we are mandated reporters. We are required by law to report suspected child abuse or neglect, and in certain circumstances, threats of harm to self or others.

Court-Involved Services: Services such as custody evaluations, co-parenting counseling, and mediation may have different confidentiality expectations than traditional therapy. Records, reports, and communications may be subject to court subpoena. Fees for court appearances, records requests, and consultations apply and will be outlined in your service agreement.

Your Rights: You have the right to inspect and copy your health information, request corrections, request an accounting of disclosures, and request restrictions on how we use or disclose your information. You also have the right to receive a paper copy of this notice.

Records Requests: To request your records, please contact us in writing at info@breezercounselingandwellness.com or by fax at 708-383-6550. A processing fee may apply.

Complaints: If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be retaliated against for filing a complaint. You may also contact the Illinois Department of Financial and Professional Regulation (IDFPR) regarding concerns about licensure or professional conduct.

Good Faith Estimate

No Surprises Act

Under the No Surprises Act, you have the right to receive a Good Faith Estimate of expected costs before receiving services. This applies to uninsured clients and those not using insurance for a particular service.

Your Good Faith Estimate will include the expected cost of services based on your specific situation. It is not a contract and does not guarantee the final cost of services. If you have questions, contact us before your first appointment at 708-303-8641 or info@breezercounselingandwellness.com.

Scope of Practice

What We Treat & What We Don’t

BCW specializes in high-conflict divorce and custody, reunification therapy, co-parenting, parallel parenting, mediation, and support for musicians, high-profile clients, and their families. We also provide individual, couples, and family therapy for a wide range of concerns including anxiety, life transitions, identity, and relationship challenges.

We do not treat active psychosis, severe eating disorders requiring medical monitoring, or conditions that require a higher level of care than outpatient therapy. If your needs fall outside our scope of practice, we will provide referrals to appropriate resources.

Cancellation Policy

Appointments & Cancellations

Session lengths vary by service type. Individual therapy sessions are typically 50 minutes. Specialty services including reunification therapy, co-parenting sessions, and custody evaluations may vary in length and will be outlined in your service agreement.

Cancellation and late cancellation policies are outlined in your intake paperwork and service agreement. Please contact us as soon as possible if you need to cancel or reschedule an appointment.
